FORM on reclamation of documents from the Kyrgyz Republic

Answers to questions should be given complete and accurate).
1. Name of the person whose document is required (if you change the name, then specify)
2. Year, month, date and place of birth
3. Citizenship (if changed, please indicate)
5. Which document is required:
a) if a document of birth, marriage, divorce, death, etc. is requested, indicate the exact place and time of registration, as well as the surname, name and patronymic of the parents when requesting a birth certificate;
b) if a document on education is requested, then indicate the name and address of the educational institution, the date of enrollment and graduation;
c) if a document on work experience is requested, then indicate the name and address of the enterprise, institution, work time and in the capacity of whom you worked; if you worked for private individuals, indicate the surname, name, patronymic and address of the employer;
d) if a document is requested for a pension granted in the Kyrgyz Republic, then indicate when, for what, and which organization appointed the pension for the last time;
e) if a criminal liability document is requested, then indicate the full and exact address of residence in Kyrgyzstan.
6. For what purpose is the document required?
7. The exact address of the place of residence, the telephone number of the person whose document is being requested.
If it is not your own document that is being claimed, then in addition, answer the following questions:
8. Your last name, first name, middle name
9. Year, month, date and place of birth
11. Your relationship to the person in whose name the document is requested.
